Submitting a Nomination

The official nomination form must be completed and must accompany every nomination submission.

Nomination material must also include:

• Nominator's Statement of Support that addresses the criteria and demonstrates achievements and contributions by the nominee, in the last five years (maximum of three pages).
• Documentary evidence supporting the nomination, e.g. testimonials, prizes and awards received, media coverage, publications, reports demonstrating outcomes of initiatives (maximum of 10 pages).
• Two written references in support of the nomination - one personal and one professional.
