Replacement testamur
A replacement testamur is a true copy of your original testamur if you have permanently lost or destroyed your original testamur.
You can request a replacement testamur for the following reasons:
- Your original testamur has been lost, damaged or destroyed
- You have legally changed your name since the issue of your testamur
What information is included in the replacement testamur?
- The testamur will indicate that it is a replacement by inclusion of the following statement: "Replacement testamur issued (date) and signed by the Academic Registrar."
- Student details such as:
- Student name at the time of graduation (except in the case of an approved change of name)
- Award title
- Conferral date
What is the cost for a replacement testamur?
- Replacement testamur due to original being lost, damaged or destroyed - $55
- Replacement testamur due to a change of name - $250
How do I apply for a replacement testamur?
Lost, damaged or stolen
Step 1: Make the $55 payment via iPay by following the payment steps.. You will be sent an email with a confirmation of your order. Please ensure you add the iPay number (IP ####) to the replacement testamur application.
Step 2: You need to complete a Request for Replacement Testamur form (PDF, 107.48 KB) (opens in a new window).
Step 3: Complete the statutory declaration
Step 4: Submit application at any Student Central or mailed to:

Change of name
Step 1: In order to apply for a replacement testamur with a new name, you must first lodge a Change of Personal Details form (PDF, 129.08 KB) (opens in a new window) along with certified documentary evidence to any UWS Student Central.
Step 2: Once the form is lodged you must complete the Request for Replacement Testamur form (PDF, 107.48 KB) (opens in a new window).
Step 3: Make the 'change of name fee' payment of $250 via iPay. You will be sent an email with a confirmation of your order. Please ensure you add the iPay number (IP ####) to the replacement testamur application.
Step 4: Complete the statutory declaration.
Step 5: Submit application at any Student Central or mailed to:

How to complete the statutory declaration?
Your statutory declaration must contain the following information:
- Name of the declarant
- How your original testamur was lost, damage or destroyed including circumstances of the loss, damage or destruction of your original testamur
- Information of the original testamur such as the conferment date and award title
- The following clause must also be included in the statutory declaration: "I undertake that if the original testamur/s is/are found I will return the replacement testamur/s to the University of Western Sydney within seven days."
Qualified witnesses for this purpose (or overseas equivalents) are:
- Justice of the Peace
- Notary Public
- Legal Practitioner (holding a current practicing certificate)
- A person authorised to administer an oath under Section 26 of the Oaths Act 1900
Important information
Postage
Your replacement testamur will be posted via registered post to the address you provide on your iPay order. Addresses from your order are stored in the system and transferred directly onto your posted item. Please ensure your address is correct as UWS does not take responsibility for lost or undelivered postage if the address provided is incorrect or if you change addresses.
Refund
If you cancel your order, there will be a $20 administration fee deducted from your refund (if applicable). To request a refund, you must email graduations@uws.edu.au providing your iPay number (IP order number, example IP234567) as per your tax invoice, your full name, your student ID number and reason for refund. Refunds will not be considered/processed unless the above information is received.
