Replacement testamurs

You can request a replacement testamur if your original testamur has been lost, damaged or destroyed.

If you have legally changed your name since the issue of your testamur, you can request a replacement by applying to the Assessment and Graduation unit. In order to apply for a replacement testamur in a new name, you must first lodge a Change of Name form at any UWS Student Centre.

The testamur will indicate that it is a replacement by inclusion of the following statement:
"Replacement testamur issued...(date) and signed by the Academic Registrar."

How to apply

Replacement testamur - Lost, Stolen or Damaged

If your original testamur is lost, stolen, damaged or destroyed you can request a replacement by applying to the Assessment and Graduation unit.

You must complete the Request for Replacement Testamur form and pay the replacement fee of AUD$50.00 via iPay.

Your application must also include a Statutory Declaration setting out the circumstances of the loss, damage or destruction of your original testamur. The Statutory Declaration provided must include specific details about your original testamur, such as the conferment date and award title.Related Links

Damaged testamurs

Damaged testamurs must be returned to the University with the application and Statutory Declaration.

Lost, stolen or destroyed testamurs

If it is not possible to state with certainty that your original testamur no longer exists, the following clause must also be included in the Statutory Declaration:

"Should the original testamur issued to me by the University of Western Sydney be returned to my possession, I undertake to return the University any replacement testamur that may be issued."

Replacement testamur - Change of Name

If you have legally changed your name since the issue of your original testamur, you can request a replacement by applying to the Assessment and Graduation unit.

In order to apply for a replacement testamur with a new name, you must first lodge a Change of Name form along with certified documentary evidence to any UWS Student Centre before you can lodge your Request for Replacement Testamur form.

You must complete the Request for Replacement Testamur form and pay for the replacement with Change of Name fee of AUD$250.00 via iPay. Your application must also include a Statutory Declaration setting out the reason for your request for a replacement testamur with a Change of Name. The Statutory Declaration provided must also include specific details about your original testamur, such as the conferment date and award title.

You must return your original testamur to the University with your application form. Replacement testamurs and replacement testamurs with Change of Name may only be issued subject to the above conditions being met.

Fee

A AUD$50.00 fee will be charged for administration and certified postage for replacement testamurs - lost, stolen or damaged.

A AUD$250.00 fee will be charged for administration and certified postage for replacement testamurs - change of name.
