IT Forms

Note: Please use Internet Explorer to access the forms. Other browsers do not support the built in features of some of the forms below, including the ability to submit the form electronically.

All forms are provided solely for UWS business purposes.

IT Access Form

ITS is working towards the auto creation of email accounts and other logins for new staff and visitors to UWS requiring access to University IT services. In the meantime, use Internet Explorer to access the IT Access Form [PDF, 195.07 KB] (opens in a new window). Complete the form and email it to the IT Service Desk by pressing Submit.

This form gathers the information required for access to IT services for staff, students and some visitors. 

Please complete the form online and email it to us using the Submit button at the base of the form.

Old, incomplete, or unsigned forms can no longer be accepted.

Email the form to the IT Service Desk (itservicedesk@uws.edu.au).

What happens next?

IT Service Desk Operators will create accounts and enable access according to the information that has been provided and approved (if required).

Service Desk or Enterprise Systems staff will contact you with your account details - correct contact details are essential.

Note: Service Desk staff do not provide details of email and network logins to anyone other than the account "owner".

Public Folder Request Form

Use Internet Explorer to access the Public Folder Request form to request a new public folder or modifications to an existing public folder. Complete the form and email it to the IT Service Desk by pressing Submit.

Computer  Software Request

Acquisition, appropriate licensing, and delivery of new software to ITS is the responsibility of the individual or school requiring the software. ITS will not be responsible for the absence of software not requested, not provided, or not appropriately licensed.

UWS Software Request Form

The UWS Software Request Form [PDF, 174.25 KB] (opens in a new window) must be emailed to itservicedesk@uws.edu.au.

All specialist software requirements should be advised - do not assume that all software will be carried forward from the previous semester. No software will be carried forward from the previous year. Only common software funded by ITS (eg, Windows, Microsoft Office, SPSS, IE, Typequick, virus protection) will be carried forward, unless otherwise advised. Version changes will only be made at the beginning of each calendar year for these common software programs. 

At the beginning of each calendar year and when new software is requested, you will be required to participate in testing to ensure that all of the functionality of the software required for your course actually works. Software will not be deployed until the testing is signed off by the requestor. Please contact Desktop Services (ext 1011 or 1012) for further information on software testing arrangements.

Note: A very small number of software products have been found to be incompatible with the current environment, so this testing is essential, and there can be no guarantee that all submitted software will work in the UWS environment.

The requested software:

  1. Must not result in multiple versions of the same software;
  2. Must not be disruptive to other software already in the UWS environment;
  3. Must be compatible with the UWS environment;
  4. Must not be otherwise disruptive to the UWS environment; and
  5. Must be submitted with sufficient lead time to test the software in the UWS environment.

General Purpose Computer Lab Software Software Deadlines

31st October is the deadline for submission of any software requirements for the next year's first semester in the General Purpose Computer Labs. The deadline for next year's second semester is 31st May. If a request is not received by the appropriate deadline, there can be no guarantee that software will be ready for the beginning of semester.

Information Architecture & Strategic Planning (IASP) Work Request

The IASP Work Request Form (opens in a new window) (MS Word, 133Kb) is used by business owners of University application systems to request Information Architecture and Strategic Planning participation in projects, resolution of application problems, application enhancements and corporate server hosting. The IASP work request is different from a request to the Service Desk.

Service Desk requests are used to manage day-to-day technical problems and incidents, rather than when an application is working, but not functioning as expected or required.

Connection to the Wired Network Form

AirUWS-Lite, the UWS wireless network, is the simplest and most appropriate method for connecting non-UWS devices to UWS networks. However, it is sometimes necessary for non-UWS or non-standard devices to be connected to the UWS wired network.

In order to maintain network security, these devices cannot be connected to the wired network without ensuring they do not present a risk to UWS. If you need to connect a non-UWS or non-standard device to the wired network, please complete the Connection of Private or Non-standard Devices to the Wired Network Form (opens in a new window) (PDF, 314Kb).

Global Roaming and International Dialling Application

IT Project Idea

A UWS project starts with an idea or a need somewhere in the UWS organisation in response to a trigger (eg business objectives, legislation changes, recommendation from an audit). Before a project can be initiated and resources committed, the idea must be assessed to verify that the project is worthwhile and viable. For projects involving technology, this process is kicked off by completing an IT Project Idea Form (opens in a new window) (MS Word, 700Kb). This form captures a high-level overview of the idea, including an outline business case and how the project supports UWS strategic objectives as defined in the UWS Making the Difference Strategy.

Guidance notes are provided in the form (MS Word must be in Print Layout View) and these should be deleted prior to submission.

Once completed and approved by the head of the business unit, the form should be submitted to the ITS Project Management Office (IASP-PMO@lists.uws.edu.au) for review and inclusion on the agenda of the next UWS IT Management & Services Committee meeting. This committee provides approval and funding for the project and prioritises the project within the UWS IT project portfolio.

Leased Asset Cost Centre Change Form

The ITP Leased Asset Cost Centre Change Form (opens in a new window) (MS Excel, 1.8MB) is used to notify ITP when a leased asset is transferred to a new cost centre.

IT Non-Leased Asset Disposal Form

The IT Non-Leased Asset Disposal Form (opens in a new window) (MS Excel, 22Kb) is used to ask ITP to coordinate the disposal of non-leased University IT assets.

New Staff Member Checklist

ITP have developed the New Staff Member Checklist (opens in a new window) (MS Word, 161Kb) to help ensure a smooth arrival for new staff. Use this form to let ITP know the new staff member's hardware and software requirements.

IT Software Request Form

Use Internet Explorer to access the IT Software Request Form [PDF, 160.6 KB] (opens in new window) to order software for staff computers. Once the form is complete, press Submit to lodge it with ITS.

Please be aware that many applications are available automatically to staff computers (opens in a new window)(Windows) connected to the UWS network, and that this form is not required to install those applications. You can find this software in your Windows Control Panel, under Run Advertised Programs.
