
Special requirements are related to enrolment in courses/units where clinical, practical or work placements are part of a course/unit structure. These include courses/units in Education, Nursing and Social & Health related sciences.
Students studying courses/units that involve working or contact with people under the age of 18 are required to complete and lodge a Prohibited Employment Declaration (PED) in accordance with the Child Protection (Prohibited Employment) Act 1998.
The PED is available electronically in MySR for commencing students. Alternatively, a PED form is available from any Student Centre. It must be completed using black pen and the instructions on the form followed carefully. This will ensure the form is processed as soon as possible.
If a PED is not completed and lodged, enrolment in the related unit/s will not proceed. Students should check their enrolment on MySR approximately 48 hours after lodging their PED. If you have any problems enrolling in courses or units which require a PED, please contact your home campus Student Centre.
If you are enrolling in a course which requires a Criminal Record Check, you will also need to submit a PED.
Students attending placement are not required to complete a Working with Children Check. Students will be required to sign a Prohibited Employment Declaration and submit it to the Placement Coordinator upon commencement of their placement.
(This is separate from the Aged Care Facility National Criminal Record Check Certificate that is required by the Department of Health and Ageing. Please note some students are required to have both checks completed.)
NSW Health requires all students attending any kind of fieldwork, research or clinical placement in a NSW Health facility as part of their course to undergo a National Criminal Record Check. You will not be able to attend your placement until this check has been completed and you have been issued a Clinical Placement Card by NSW Health.
How to complete the Check
1. Complete the National Criminal Record Check Consent form. The National Criminal Record Check Consent Form is available electronically in MySR for commencing students.

If you can not complete the form electronically, a National Criminal Record Check Form is available at any Student Centre.

2. All students who complete a National Criminal Record Check Consent Form must satisfy the 100-point ID check. If you complete the consent form on MySR, you must attend a Student Centre with at least 100 points of supporting identifying documentation. The Student Centre Officer will register your 100-point ID check on your student record.

3. Payment of appropriate fee. NSW Health charges a $30 fee to conduct the check. You can pay this fee via the University’s IPAY system.
Please note that your details will not be forwarded to NSW Health until both the payment AND the 100-point ID check are complete.
Failure to provide appropriate documents for the ID check or make the payment will result in your application for a NSW Health Clinical Placement Card being rejected, and you will not be able to attend any 'placement' units. This may result in you being unable to complete your enrolled unit/s, or your course.
If students require any subsequent clearance checks, a fee will apply. The application form is available from NSW Health.
All applications screened by NSW Health are returned to the University coordinator within 4-6 weeks from the submission date. UWS will post your card to the local address you have provided in MySR.
All students who complete a National Criminal Record Check Consent Form must also complete a Prohibited Employment Declaration form. This form is also available on MySR, or from a Student Centre.
NSW Health requires ALL students who will be entering their facilities as part of any fieldwork, research or clinical placement, to produce evidence of current immunisation or vaccination against infectious diseases.
Details regarding the Vaccination requirements are included in the NSW Department of Health PD2007_006 Occupational Assessment, Screening & Vaccination Against Specified Infectious Diseases. Vaccination record cards are available at Student Centres.
International students should have their overseas immunisation records assessed by a NSW doctor. Screening and vaccination done overseas may be recognised in NSW subject to the following conditions:
Note: It is your responsibility to ensure you meet any vaccination requirements prior to attending fieldwork, research or clinical placement and that your card is available for NSW Health supervisory staff to view as required when attending your placement.
Failure to meet vaccination requirements could result in you being denied entry to a NSW Health facility and will jeopardise your progression in the associated unit.
Students who are required to have a current First Aid certificate for participation in particular units and/or fieldwork, research or clinical placement will need to:
Take their original current First Aid certificate (from a recognised WorkCover NSW accredited provider) to a Student Centre with their current Student ID card or photo ID (eg, a current Drivers Licence or passport).
Have a Student Centre Officer check their credentials and process the requirement on their student record.
Check MySR approximately 48 hours later to ensure enrolment in the relevant unit has been confirmed.
Note: You cannot enrol into the associated unit until this requirement has been validated.

The Department of Health and Ageing require all students attending any clinical placement in an Aged Care Facility to have a National Criminal History Record Check.
How to complete the Check
1. Residents of NSW are required to apply through the NSW Police, using form P799. The form can be found on the Police website. Students will need to contact the NSW Police station nearest your place of residence to organise a time and location to submit the application form.
2. Present application form and three (3) types of acceptable identification to the nominated police station at the allocated time.
3. Pay the appropriate fee upon submission of the form to the NSW Police.
Fees as at March 2006 are ‘Name and date of birth check’ ($52)
You will need to apply in an appropriate timeframe to allow you to receive the certificate prior to your placement (approx 3 weeks prior).
4. Present your certificate to your Clinical Coordinator within the School.
5. Present your certificate to your nearest Student Centre to have this recorded on your record.
You will also need to provide your certificate to the Aged Care provider that you are completing your placement with.
